Long Term Conditions (LTC)

Levin Family Health offers a variety of programs to assist patients with chronic or long-term conditions such as diabetes, asthma, COPD, heart problems, gout, depression, and more. Patients will receive an initial full assessment, an individual care plan, regular follow-ups for the effective management of their condition, and support for lifestyle changes.

Self-management support programs, including health coaching, are available to eligible patients for free or at a subsidized rate. Self-management education programs are also available in the local community to improve wellbeing and help individuals learn new skills to manage their health condition. The program also helps individuals develop confidence in managing their condition, meet others with similar experiences, and achieve personal goals.
